Please read following instructions carefully before filling and submitting your application form for the Elective Program.
STEP 1
Make sure that it is compulsory to have one referee from the same specialty that you have chosen for your Elective (as your choice of preference) and the second reference letter can be filled by any of the faculty member working at KKUH.
Now move to STEP 2

- Ensure that you are using Chrome or Safari browsers to better view the forms.
- Make sure you have scanned the 2 reference letters and your Transcript (academic record ). As later on these would be required to be uploaded at the time of submission of your application form for the Elective Program.
- Select the elective progam ( Selective Elective or Summer Elective ) and fill it.
- Make sure to print your application form (after submission, you will have an option from your screen to do so) and keep this with your originals, to show them if requested. Ensure that your printer is plugged in before filling the forms.
